Application Process

Our application process has four sequential steps in advance of enrollment. At each step, we will work with you to obtain key information necessary to determine eligibility for enrollment. All information collected at each phase remains confidential and data is managed in accordance with FERPA and HIPPA regulations. 


Step 1 - Student Interest Form

As a first step, please fill out our school Interest Form and we will contact you.

OR

Attend an Information Night to tour the facility, meet the MCP staff and schedule a personalized tour and consultation meeting.

Information Nights take place from 6:00-7:30 pm at 125 Bethany Drive, Scotts Valley CA 95066. See our Information Night Schedule for current dates and to RSVP.


Step 2 - Personalized Tour and Consultation Meeting

During the meeting you and your student (if appropriate), will observe classes, discuss your student’s educational needs, and schedule a shadow date for your student.

Step 3 - Student Shadow

To determine if MCP is a good match students are required to attend MCP, meet fellow students and participate in classes. Some students may come for part of a school day at first, and some make multiple visits in order to become gently familiar with our community.


Step 4 - On-line Application 

After the shadow date we encourage families to complete the application for enrollment and submit required documents.

Complete the on-line application, or request a paper application packet from pmassaro@exploremcp.org.

After we receive the completed application and required supporting documents we will make an enrollment determination and send you written notification of acceptance.

SUPPORTING DOCUMENTS

In order to help us determine eligibility, please submit as many of the following documents as you have available. Please note: Due to HIPPA and FERPA most professionals and schools will not release information without your signed consent.

  • Current IEP or 504 plan

  • Triennial IEP testing results and assessment reports

  • Private psycho-educational assessment

  • Physician diagnosis or reports

  • Speech and Language Evaluation

  • Educational Therapist Report

  • Any other documents that may help us to understand your student’s learning and educational needs/history.

You may submit forms by:
